Here is a list of the documents needed for repatriation to Zimbabwe from South Africa. Tanaka Life  can help you  obtain all the necessary paperwork if your funeral parlour is unable.

 There are lots of companies and individuals that do repatriation to Zimbabwe and all have to follow the procedures below as indicated by the  Zimbabwean Consulate

Requirements for Repatriation to Zimbabwe

  1. Abridged Death certificate     You can get an abridged death certificate at your nearest home affairs  office. You need to complete and submit the notification of death (Form BI-1663)and the Death Report (Form BI-1680) to get an abridged death certificate. Home affairs will issue the document there and then.

  2. Copy of Deceased’s  Zimbabwean I.D     Each government wants to know who is entering their country . You will be asked to produce a copy of the deceased’s passport, birth certificate, Zimbabwean ID to ensure they are a Zimbabwean citizen.

  3. Copy of Deceased relative’s identity     A copy of the next of kin’s identity is required as well. This could be a passport, Birth Certificate or Zimbabwean Identity document. The relative should also provide their contact details.
  4. Burial Order    South African law states that no person may be buried without a burial order. A burial order may be issued by funeral undertaker who is permitted and recognised by law. It can also be issued by an SAPS official or specific home affairs official. 
  5. Embalming Certificate – An embalming certificate should clearly state the remains were embalmed, sealed in an airtight container and placed in a sturdy non-transparent coffin in the case of non-infectious remains. For infectious remains, the embalming certificate should clearly state the remains were embalmed, placed in a polythene bag, placed in a second polythene bag, sealed in an airtight container and placed in a sturdy non-transparent coffin.
  6. Non-Infectious disease Certificate
  7. Notice of Death/Still Birth – (DHA-1663A) a form that has particulars of the deceased. It has sections that must be completed by the attending medical practioner/professional nurse, forensic pathologist, informant and funeral undertaker
  8. Notice of Death/Still Birth – (DHA-1663B)-confirmation for medical and health use only
  9. Final burial place in Zimbabwe- Lastly you need to indicate final resting place in Zimbabwe. Tanaka Life is able to do repatriations to anywhere in Zimbabwe.

  1. wich mode of transport do you use for repatriation?and where is your palour based in jhb?ls it where we do our last service before leaving for designated country?

      We use road transport for repatriation. We are based in Johannesburg. Since we are based in Johannesburg and service the whole of South Africa we normally use a chapel around the area the deceased used to live for the memorial.
